When new patients visit our office for the first time there is some paperwork that needs to be filled out. Please bring any pertinent tests or medical records on your first office visit.

Billing & Payments

A list of standard charges is available in our office. Billing is done in-house and we process insurance forms for your convenience. Once you have given our business office all of your insurance information, our staff will submit the claims electronically.  It is the patient’s responsibility to update the office with the most recent insurance information, to pay deductible amounts and other balances not covered by your insurance companies.

We accept most HMO’s and insurance programs for care that is covered by insurance. However, in many cases, office visit fees are not covered by health care plans. You are responsible for physician fees that must be paid at the time of the visit. Cosmetic surgery fees are generally not covered by insurance carriers and must be paid at least two weeks prior to scheduled surgery. Surgical procedures performed in the office, in an outpatient surgical setting, or in the hospital will be billed to your insurance carrier. Any unpaid balance will be your responsibility. Should financial hardship exist, our billing office will work with you to reach a satisfactory solution.

We know that questions may arise on insurance matters. We encourage you to discuss such questions with our office staff. We will be happy to help you receive the maximum benefit.
